Before taking an Aids there is a form, what questions are on this form?

Is there a form that asks for personal information such as how many people have you had intercourse with in the last _____?
and are you ever asked to name those people?

no,they just ask you how many within the last 3 weeks

NO - the reason that question is asked is that GOD forbid you test positive, the person who counsels you will advise you to tell that number of previous partners. But, no one would ever ask names and, by the way, I believe you can even take the test annonomously but not 100% sure on that.

I was only asked if I had ever had unprotected sex and if I was an IV drug user. Thats was it. If you go to your local health clinic they will do a little educating to you. But regardless its all worth it in the end.
