How can I design a knowledge assessment tool for a community on isues of HIV and AIDS? Thank you.?

I would like to come up with a knowledge assessment tool in order to know how much the community I am working in know about HIV/AIDS. This would help me better understand how best to design and present educational messages on the same

Create a simple survey of questions about HIV, no more than a page long and written at a 6th grade level. Use multiple choice questions. Cover important topics such as mode of transmission and prevention. And collect demographic information (age, sex, race, sexual orientation).

Score the surveys and see how the scores relate to the demographic info you have. Is there less knowledge in a certain ethnic group or orientation group? Who is at risk. Compare your results to the previous literature. Then figure out to which audience you want to target your message.
