I need help with a poem.?

I need to create a poem about PCP/angel dust. It has to include the name (PCP or Angel dust), at least 3 accurate effects of the drug, and at lest 2 consequences of using the drug. PLEASE help. Im really stuck.

Some effects include:
Tolerance
Depression
Anxiety
Flashbacks
Kidney failure
HIV/AIDS
Seizures
Coma
Blurred Vision
Sweating
Vomiting
Dizziness
Numbness
Eye Fluttering
Muscle spasms

Some consequences:
Death
Addiction
High blood pressure
Hepatitis
Weight loss
Birth defects
HIV/AIDS
