Hi, what is the best hiv drug so far? is it possible for a patient to marry and have kids? if yes how?

There is no one drug that is "best". It depends on the strain of the virus that people have, as some strains are resistant to some medications. There are also a variety of classes of drugs that act on different aspects of HIV transmission.

Yes, it is totally possible to have children. If the sperm donor is HIV+ then a process called sperm washing can be done at some family planning clinics followed by artificial insemination. It is safe and has been tested on more than 5000 occasions with no transmissions of HIV to the women.

If the women is positive, she can again have artificial insemination. SHe would be started on HIV meds (regardless of whether she needs it for her own health) at around 14-16 weeks gestation, followed by IV HIV drugs at delivery, and possibly a C-section, and the child would be on a syrup form of HIV meds for about a month.....all of which can reduce the likelihood of passing it on to her child to around 1% or less.

Because HIV generates resistance to medication and a resistant strain can be transmitted with that quality the best drug is the one which the patients HIV is not CURRENTLY resistant to. DNA/RNA tests can be done to determine the best therapeutic agents. There are very few if any good monotherapies, and with people living longer and going through all the first line medications the drugs of choice and the best drugs are 'whatever is the newest drug' because it's salvage for 10 years now. I'd say entry inhibitors are up there as a good advance, but it's a tough one.

Marriage is a social contract, not a medical condition so depending where you live you can most likely marry somebody of the opposite sex with HIV (generalization not prejudice there).

Now I'll deviate a bit from the norm. Sure, you can be in a serodiscordant marriage or both people can be positive and want a kid but there is no cure and most people are going to die from the side effects of the disease later. It's a risk. I put myself in the child's place. I would not want to be brought into a world by parent's who knew they were more likely to die than most other people. Just because AIDS isn't the leading cause of death in an age group doesn't mean that it cancels out the risk factor. If you don't die from AIDS and die from a heart attack you still die and carry the risk.

Was NOT BREASTFEEDING even mentioned?

Seriously, if you have a kid no amount of syrup is going to magically stop them from catching HIV when they are living from HIV positive bodily fluids. NO BREAST FEEDING IF HIV+ MOTHER.

I apologize if I appear to have an uneducated or prejudiced opinion, but I don't think people with HIV should conceive children for many reasons. Be honest, how would you feel if there was an accident and your child contracted HIV from you?

That said, as the above poster stated sperm washing works.

If you aren't having kids an are serodiscordant then PrEP (per-exposure prophylaxis) with anti-retroviral medication can significantly reduce transmission to the negative partner.

HIV is not a death sentence, but failing to modify behavior because people feel good is a major reason for a) non-compliance and failure with medication and b) further spread of the disease.
