Do you think that the category of race/ethnicity or age/sex is important when gathering statistics?

This is not to say whether or not you are racist, agist or sexist... Only whether or not you think it's important to gather data by categorizing people.

For instance, do you think its important when studying HIV to know that black women between the ages of 22-30 have the highest increase in rates of infection?

Do you think it's important, when studying employment that black women between the ages of 40-60 show the highest rate of job stability - staying at their jobs for much longer than any other group?

Does this really tell us anything useful or is it just confusing the issues?

Does it encourage prejudice?

The statistics above may not be completely accurate... I was just reciting them from memory and don't really have a link or anything...

Categories give us more information than we would have had. It is useful in trying to explain and address issues and problems.

Depends on how the categories are used obviously but extra information in itself is always positive.
Interpretation and honest analysis is everything or objective facts can be turned into tools to justify prejudice and outright discrimination.

Example there are more young black men in jail in the States than in college. That's a fact yet would that justify saying blacks are simply more criminal as some racists would probably do? Of course not but it's still essential information that indicates black people are still disadvantaged in today's America. That's necessary to point out the need for measures addressing that issue

Honest interpretation of statistics is everything. Factual information is not the problem

Many times yes, it is how we can see where problems are, who is at risk of many health problems, like diabetes, heart disease, brest cancer. So it really does help !

It really depends on what the topic your polling about is.
