Could a couple with hiv still have a sexual* relation with each other ?


Would their infection gets worse if they do?

Hi there,

If both partners in a couple were hiv+ and it was a monogamous relationship, as consenting adults, some may choose to do away with condoms and other safer sex guidelines. The decision to do so is not an easy one as there is the chance of reinfecting each other. Some factors that the couple may want to evaluate are the viral loads (the amount of the virus present in the blood); the lower the better: "undetectable" or <50 copies per millilitre of blood is the goal of prophylaxis. The couple may also want to make sure that they do not have any other stds which they could pass on to each other.

The danger with reinfecting each other is that it is possible that in reinfecting each other, present drug regimens may no longer work as the strand of the virus and any drug resistances would be passed to the partner. That may not sound like a biggie but when there are limited number of drugs available to begin with and drug resistances are usually across a whole class of drugs (as opposed to one specific drug), it is possible to essentially wipe out any or all remaining drugs to use.

Now, if the couple choose to continue to follow sefer sex guidelines, the sky's the limit! Their infections will not "get worse" simply by having safer sex; more risky behavious could possibly make things worse for them.
