Where do I apply for SSI and Medicaid??? Can I do it online??

I am 20, HIV+, just got out of the hospital Sunday, am uninsured, and I need financial assistance for myself and to help pay bills and cover prescriptions because i've been unable to work for quite sometime and I need the assistance. If you all could give me names and numbers I would greatly appreciate the info.
