Case management at ASC in Pasadena or APLA Mid Wilshire has answers and help. If you have income from SSDI or SSI, or even if you have no income, there are programs to give you grant $$'s to move into an apt. Personally, unless you are too far west in L.A. County, I recommend ASC. I am a client there.

In the United States there are government programs. Contact your local state Division of Vocational Rehabilitation Services to get help with a job.

If you have the advantage of living in a mid-large city, check the yellow book "HIV/AIDS Resources." In Orlando, for example, there were over 10 agencies (not for profit/non-govt) who provided different kinds of help. Get connected with a local resource group (even if that's an hour's drive) can help with housing, jobs, meds, getting a good doctor, having a support group can make all the difference in your life. Sending you clear white strength and energy, plus the desire to help yourself.
