Be safe, don't have sex with an HIV-infected patient. Period. There is blood in the mouth from gum disease and poor oral health care. Not everyone brushes twice a day, flosses, and rinses with mouth wash like they are supposed to. Even if they do, periodontal (gums, tongue...) disease is often one of the -first- visual signs of HIV.

There is a chance if you do not use condom. The risk is wounds in her/his mouth and on your penis. There is very low hiv concentration in saliva though, but hey - risk is still there.
