CA State Disability Question?

I was in the hospital Sept 25-Oct26 for Pneumonia and I found out that I am HIV positive. right now in can not walk ( I am using a walker) I am not sure when I will be able to go back to work. Do I qualify for disabilty? If so, what do I need to do? Does my doctor need to fill out the form?

There's a good chance. If you apply now, you will need to show that you worked at least one quarter between July 1, 2006 and June 30, 2007. Your weekly pay will be based on how much you earned in your maximum earning quarter during that period.
A doctor will need to "certify" that you are unable to work due to disability. This is done by his completing and signing the state form.
You will only get disability for 6 months, although the state does allow extensions for another 6 months occasionally.
I hope your condition improves but, if not, you should start thinking about applying for Social Security disability which does not have a 6 month limit.
Remember California state disability is a state program and relies on work quarters going back approximately 1 year
Social Security disability is a Federal program based on working quarters during the past 10 years.
California state disability is not taxable unlike state unemployment benefits which are taxable. Disability benefits are considerably larger that unemployment benefits

Anytime you are off work due to illness, you should be able to get disability. You can get the forms online and yes, your doctor has to fill out a section on the form(s) as do you.
