Swollen scrotum??? What could cause this?

I travel to Thailand on a regular basis for work. I have been 20 times. For the past 4 trips, when I returned home I would get these itchy red bumps on my butt and my scrotum would get very swollen. I went to the doctor the first time it happened and she told me I had a staph infection and gave me antibiotics. I'm thinking there is something that I come in contact with that is giving me some sort of alergic reaction. I have been with some of the prostitutes, always used condoms, but I have been tested for HIV several times and they all came back negative. Any idea what this could be?

Maybe it's just the general hygiene of the place, what about the toilet seats etc you use their. It could be anything.

Condoms don't cover your scrotum. That could be it. My advice is stay away from the prostitutes.
