This Nurse Lied on My Medical Record. How could she have Checked My Thyroid?


I went to Planned Parenthood to receive treatment for the STD I caught from a sexual assault. Long story short..she completely botched the procedure and I am having corrective surgery soon to attempt to correct the damage.

Anyways, she lied many times on the form.....writing down that she lectured me on "safer sex, smoking risks (I don't even smoke, HIV risk reduction, etc.

She wrote down she gave me breast self exam instructions when she never did.

Also, there are the words "heart" "lungs" and thyroid" with a space next to it. She checked off all of those items.

My quesiton is: What procedure does it take to check those items? She never did ANYTHING to me above the waist. She didn't check my heart, lungs or thyroid. Would that ential using a stethsecope to listent to my heart and lungs??

How would a thyroid be checked?

Should I go to the medical director with my concerns?

She put acid on my entire vulva so it ate through the tissue of my skin. They are supposed to just put the acid on the condyloma (the STD) itself.

I asked her if there were any risks and she said "No"

What kind of corrective surgery do you need to correct the damange from STD treatment?? I'm confused. What the heck did she do to you?

I would take your concerns up with the planned parenthood clinic you went to.

Thyroid check needs (a) she feels your thyroid (lower neck region) for swelling and (b) a blood test to check hormone levels.

Why is a nurse doing these type of medical procedures? It needs a doctor's qualifications as far as I know. Complain if you have proof she said she told you stuff that she didn't. She's a slacker.

I believe you should, because this nurse was cutting corners with her job, obviously to lazy to do her job properly, She should be reported.

A basic thyroid check involves palpating (feeling) the thyroid. This is usually done with the examiner behind you, with both hands feeling on either side of your neck. They are feeling for swelling, nodes, or crepitus (crackly fluid build up). A stethescope would also be used to listen to blood flow over the thyroid, as some problems give an audible sound ("bruit").

Heart and lungs would be checked with stethescope, as well as percussion (tapping) on chest and abdomen, as well as some visual checks (eg. looking for swelling, or "sucking in" between ribs, etc.)

If she did not examine you, you should indeed go to the medical director. You may also want to consider going to your nursing governing body (eg. College of Nurses). Lying on a chart, or charting information on physical tests that were never done is a very serious ethical matter. This in contrary to ethical and responsible nursing practice.

(ETA: Moysant1 - conducting physical examinations is well within the nursing scope of practice, although it is something often done by doctors. Nurses can not diagnose illness per se, through this, but are well within their scope to chart and detail anything noted on physical exam, and are also allowed to provide nursing diagnoses and course of action related to that. Depending on the nurse's qualifications (eg. Nurse practioner), she may also be able to diagnose, treat, and prescribe independent of a doctor. This is not unusual practice at all in some clinics and hospitals.
